This service includes a comprehensive evaluation with a licensed mental health professional to assess your current symptoms, mental health needs, functional limitations, and how your condition may be affecting your ability to perform your work responsibilities.
Your service includes:
Comprehensive mental health evaluation
Assessment of current symptoms and functional limitations
Discussion of continuous or intermittent leave needs
Completion of appropriate FMLA documentation when clinically supported
Treatment recommendations and referrals, when appropriate
Eligibility: This service is available to clients currently residing in Virginia or Texas.
Important: Purchasing an evaluation does not guarantee that FMLA documentation will be completed or that leave will be approved. Documentation is provided only when clinically appropriate and supported by the evaluation. Final FMLA eligibility and approval are determined by your employer and/or FMLA administrator.
